New Doctor and Prescription Lookup Widgets Are Live on Your Health Quote Forms and Surveys

Your Health Quote Form and Health Quote Survey inside Max Methodology just got a serious upgrade. We've rolled out new widget based versions of both, built to work the same way you're used to, but with smarter fields and two brand new lookup tools built right in: doctor and provider search, and prescription search. These are duplicated versions of your existing forms, so nothing you've already customized gets touched. You'll just need to swap the new versions in once you're ready. Here's everything that's new and how to turn it on.

What's New

Two new widget based forms are now live:

  • Health Quote Form

  • Health Quote Survey

Both are duplicated versions of the originals, so any custom edits you've made to your existing forms stay exactly as they are. The workflow is the same one you already know, just with upgraded fields and two new lookup widgets built in.

Smarter Questions for Better Data

We added several new questions based on direct feedback from agents:

  • Dependents: now asks whether dependents are claimed on taxes

  • Marital status: selecting "couple" splits first and last name into separate fields

  • Gender: added to both the quote form and quote survey

  • Coverage type: asks whether the client is looking for marketplace, health insurance, a short term alternative, or isn't sure yet

  • Employer coverage: asks whether the client was offered coverage through an employer, even if they declined it

These updates help you nail down subsidy eligibility faster and improve the overall quality of the data you're collecting.

Doctor and Prescription Search, Built Like HealthSherpa

The two new widgets work a lot like HealthSherpa, right inside your form.

Doctor and provider search:

  • Enter a zip code

  • Enter a first name, last name, or both

  • Choose a provider type like primary care or specialist

  • The widget returns matching providers, including MPI details

Prescription search:

  • Enter the medication name

  • The widget pulls up the drug and its available formularies

  • Review and confirm the list with your client

Fields like who takes each medication, additional coverage, and main priority are still there and work the same way. Once the HealthSherpa integration is complete, these selections will map directly into HealthSherpa.

Where to Find the New Forms and Surveys

  • Quote forms: Quote Management → Health Quote Widgets

  • Surveys: Quote Surveys → Health Survey Widgets

The Health Survey also got a layout refresh. It's now split across multiple pages, which makes it easier to work through.

How to Turn On the New Widgets

Activating the new widgets means swapping the old form or survey out for the new one in three places. Your URLs and funnel structure stay exactly the same, you're only changing which form is loaded.

  1. Calendar Appointment Funnel: Open the funnel, go to Step 2, and replace the existing Health Quote Form with Health Quote Widgets. Save and publish.

  2. New Survey Funnels: Replace the existing Health Survey with Health Survey Widgets. Save and publish.

  3. Quote Forms: Go to Quote Forms → Health Quote Form and replace it with the new widgets version. Save and publish.

A Quick Note on Button Colors

There's custom CSS already built into these forms that controls button color, which is why your submit and next buttons might show up purple in the builder even if they appear blue elsewhere. If you want to change that, update the CSS, or remove it entirely if you'd rather use the default styling. Just don't leave the old CSS in place if you're expecting a different look, it will override what you're trying to change.

Where Your Data Lands

  • New questions are stored in the household and health quote folders

  • Provider and medication results write directly into the Provider Summary and Medication Summary fields as bulleted lists

  • The old spouse name field still exists but is now marked deprecated

  • Dependent date of birth fields 1 through 5 have moved from free text to date pickers. The old fields are still there and marked deprecated so your existing data stays visible

A separate walkthrough will cover how to reconcile old and new fields once the new contact module rolls out to help map them automatically.

Test Before You Launch

Once you've swapped everything in, run through the forms and surveys yourself. Confirm the widgets pull correctly, the new questions show up, and the summaries populate the way they should. If anything looks off, let us know.
